In order to plan second-line treatment, another liver biopsy was performed, which indicated postnecrotic liver cirrhosis (Figs 5 and 6). At this time, liver tests were within the normal range, apart from slight elevation of alkaline phosphatase (155 U/L, ULN < 104 U/L).

Postnecrotic (micronodular) cirrhosis consist of broad bands of scar tissue and results from previous acute viral hepatitis or drug-induced massive hepatic necrosis. Biliary cirrhosis consists of scarring of bile ducts and lobes of the liver and results from chronic biliary obstruction and infection (cholangitis), and is much rarer than the preceding forms.

Postnecrotic cirrhosis

McGraw-Hill Dictionary of Scientific & Technical Terms, 6E, Copyright © 2003 by The Twelve of the 21 (57%) alcoholic cirrhotics and 16 of the 27 (59%) postnecrotic cirrhotics had a history of impotence. Both alcoholic and postnecrotic cirrhotic patients had significantly lower basal testosterone, but higher estradiol and prolactin levels than the control group (p less than 0.05). Postnecrotic cirrhosis is characterized by units of various sizes, separated by connective tissue fibers. Macroscopically liver resembles a bunch of grapes.
